Our Medical Spa is open, and we are committed to doing everything we can to keep you safe and protected. When you walk through out doors, we want you to know that your health and well being are our #1 priority. To keep our staff and patients safe we ask that you consider the following before arriving at your appointment. If you have answered yes to any of the following, please contact us at 858-259-4772 to reschedule your appointment.

Have you experienced any of the following symptoms in the past 48 hours?

• Fever or chills
• Cough
• Shortness of breath or difficulty breathing
• Fatigue
• Muscle or body aches
• Headaches
• Loss of taste or smell
• Sore throat
• Congestion or a runny nose
• Nausea or vomiting
• Diarrhea

Within the past 14 days, have you been in close physical contact (6 feet or closer for a cumulative of 15 minutes) with:

• Anyone who is knows to have a labratory- confirmed COVID-19 result?
• Anyone who has any symptoms consistent with COVID-19?

Have you recently done any of the following?

• Are you isolating because you may have been exposed to a person with COVID-19 or are worried that you may be sick with COVID-19?
• Are you currently waiting on personal results of a COVID-19 test?
